Gleneagles Close has three bedrooms in a shared house with a communal bathroom, kitchen and lounge. We provide long-term supported housing for adults aged 18 and over, with low to moderate learning disabilities.

Staff draw up a personalised support plan with residents using the Life Star model to help residents achieve their aims and goals.

Staff availability

Our highly trained staff are on-site part-time during office hours to deliver personalised support and meet residents’ needs. There is an out-of-hours telephone service, should residents require any further support.

  • Dealing with correspondence
  • Socialising and community involvement
  • Maintaining health, safety and security
  • Signposting and accessing other services
  • Building confidence, resilience and self-esteem
  • Maintaining a tenancy

  • Cooking, cleaning and shopping
  • Maintaining health, safety and security
  • Maintaining appropriate behaviour and communication

Additional support services

We also provide follow-on support when residents move on to live independently. Residents can purchase telecare technology to reduce their need for support whilst still feeling safe.Care and Support

Eligibility

To be eligible for a place at Gleaneagles Close, applicants must:

  • Be aged 18 or over
  • Have a low to moderate learning disability
  • Have a minimum of 10.5 care and support hours per week

How to apply

We accept referrals from the Adult Learning Disabilities team Cheshire West and Cheshire Council. All support needs will be assessed before an application is accepted.
