Woodlea Avenue has eight ensuite rooms in a shared house with a communal kitchen and lounge. We provide long-term supported housing for adults aged 18 and over, with low to moderate learning disabilities.

Care and support planning

Staff draw up a personalised support plan with residents using the Life Star model to help residents achieve their aims and goals.

Staff availability

Our highly trained staff are on-site during office hours and part-time at weekends to deliver personalised support and meet residents’ needs. There is an out-of-hours telephone service, should residents require any further support.

  • Maintaining appropriate behaviour and communication

Additional support services

We also provide follow-on support when residents move on to live independently. Residents can purchase telecare technology to reduce their need for support whilst still feeling safe.

To be eligible for a place at Woodlea Avenue, applicants must:

  • Be aged 18 or over
  • Have a low to moderate learning disability
  • Have a minimum of 10.5 care and support hours per week

How to apply

We accept referrals from the Adult Learning Disabilities team Cheshire West and Cheshire Council. All support needs will be assessed before an application is accepted.
