Support planning
Staff assist clients to complete a flexible and personalised support plan using the Outcomes Star model, to identify their personal goals and take the steps needed to become more independent. Our support is tailored to each person’s individual needs and can include telephone support and one-to-one support sessions in the home or local community.
Staff availability
Our highly-trained staff can provide support five days a week, during office hours. Support levels can vary to suit the client’s needs, from just a couple of hours a week, to more frequent and regular levels of support. We are also able to offer some out-of-hours support for an individual if required, if previously agreed with the relevant Council's Housing Options team.
We can support with
- Managing housing-related issues such as homelessness, evictions and re-settlement
- Securing and maintaining a tenancy
- Transitioning to a new tenancy
- Moving on from supported accommodation
- Finding suitable accommodation
- Applying for accommodation via social or private landlords
- Signposting and accessing other services
- Managing finances (budgeting and benefits)
