In partnership with Hampshire, Southampton and Isle of Wight CCG and Southern Health Trust, The Lookout uses a holistic, therapeutic approach to proactively support adults aged 18-65 who require intensive mental health support, through structured activities and interventions in a safe space.

Staff are on-site 24-hours a day, out-of-hours telephone service and CCTV help to ensure residents’ safety and security.

Support planning

Staff provide support as set out in the Crisis Resolution at Home Treatment Team’s Care Assessment Plan and Care Safety Plan.

Our experienced team design structured activities and interventions in a safe space. Pressures and stresses of daily life are minimised to give clients the opportunity to reset.

Staff availability

Our specialist staff are on-site 24 hours a day, seven days a week, giving clients peace of mind that support is always on-hand. There is also an out-of-hours telephone service, should clients require any further support.

Eligibility

All support needs will be assessed before an application is accepted.

To be eligible for a place at The Lookout, applicants must be over 18 with or without a mental health diagnosis and be a client with the Hampshire Community Mental Health Teams (CMHT) via the Crisis Resolution at Home Treatment Team (CRHT).

How to apply

We accept referrals from GPs, Wellbeing Centres and Hampshire Community Mental Health Teams (CMHT), but only via the Crisis Resolution at Home Treatment Team (CRHT).
