Welcome to our supported accommodation service! We're here to provide a welcoming, safe, calm and supportive space for young people aged 16-17 like you. Our main purpose is to make your time with us a happy and enjoyable experience. Whether you're preparing to return home or move on to work, college, or university, we'll assist you in planning and preparing for your next steps.

We have a dedicated team who are here to help you make the most of your stay. Even if you've faced difficulties in the past, our team is ready to work with you and support you in reaching your aspirations.

Remember, you're not alone in this journey. We're here to support you every step of the way and help you create a brighter future.

What to expect

When you move in with us you will be part of a community where you can access the support you want to achieve goals and live happily.

You will have a Keyworker who will work with you to develop a support plan and safety management plan. Your input matters to us, and we strongly believe in involving you in everything we do. We want you to take the lead in your support planning to make sure the goals you are working towards are what you want and that you know how we will all work together to keep you safe.

During your initial weeks here, we'll assist you in registering with a doctor, dentist, and optician. We want to ensure you have access to essential healthcare services, as well as help you familiarise yourself with the local area, making it feel like home.

We work alongside a fantastic team of Project Workers who will work with you as Keyworkers. They will run support sessions and activities for you to get involved in. All of our team are looking forward to getting to know you and start to support you.

Your Keyworker

Your keyworker is a member of our team who will work with you to ensure you are supported and safe.

They will check in with you regularly and set up support meetings at a time that works for you so that you can review your progress and set goals that you would like to work towards. They will work alongside your Social Worker and attend meetings with them to support you. Your Keyworker will be able to help you access your entitlements regarding housing, health and travel.

Your Support Plan

During your time with us we will work with you to create a support plan that works towards the goals you want to achieve. Your plan is yours, if there is something you want to work on let us know! We will work with your social worker to build your support plan and update your pathway plan.

When we start working with you we will:

  • Listen to what you need and want to help you achieve your goals.
  • Work with other people that will support you.
  • Talk to you about any changes that may affect you and help you speak with an advocate.
  • Agree a plan with clear actions.
  • Review your plan with you regularly to make sure it is still what you want.
  • Keep records of your meetings, goals and safety plans.

To make sure that the support is working for you, we need you to meet with your Keyworker regularly to review and agree the support you want.

We may end your support and housing agreement if:

  • You no longer need support.
  • You do not want to work with any of the team.
  • The funders choose a different provider.
  • Your tenancy has ended.
  • You cause a serious risk to the team or other residents.

Your support plan can contain goals you would like to work on about any area of your life but will usually include information about:
