Support planning

We offer a wide range of free support to people diagnosed with dementia including their family and carers.

Staff assist clients to create a personalised support plan, to identify their personal needs and plan the steps needed to live their life to the full for as long as possible by maximising their independence while living with their diagnosis.

We focus on our clients’ wishes and needs.  Through a combination of group activities and one-to-one support, we aim to promote self-confidence and empower clients to take control of their lives. We support people in their own homes and in the community, through regular drop-ins, telephone support, workshops, group activities and one-to-one support meetings. All support is flexible and personalised to individual needs.

Staff availability

Our highly-trained, friendly and welcoming staff provide support five days a week, during office hours. We provide weekly contact, but this can vary to suit the client’s needs.

We can support with

  • Getting practical advice and assistance with paperwork 
  • Creating Power of Attorney and Court of Protection applications 
  • Handling housing-related issues, such as adaptations
  • Managing finances (budgeting and benefits)
  • Planning in case of emergency
  • Creating a ‘Life Story’ booklet 
  • Creating an ‘All About Me and My Care’ booklet 
  • Accessing other support and respite services
  • Getting a Statutory Carer Assessment and acquiring a Carer Emergency Card
  • Socialising and community involvement

We offer a range of regular events and activities throughout the year, including a summer day trip during Dementia Awareness Week, World Alzheimer’s Day event and a Christmas party. Forget-me-not Café

The Forget-me-not Café (PDF 154KB) is held at Ormesby Table Tennis Club, every Friday from 1pm to 3pm, and is supported by our kind and caring staff.

Join us for a range of dementia-friendly activities, including table tennis, mindful movement, dance and lots more. Contact us for more information, or just come along on the day.

Dementia Support Group

The Dementia Support Group meets every fourth Wednesday of the month, and gives you the opportunity to socialise and enjoy a range of activities designed to be fun and mentally stimulating. Our support group is free to attend and no prior booking is necessary.

Quarterly Focus Group

Our focus group meets in January, April, July and October, on the fourth Wednesday of the month. It offers our members a chance to have their say about the services and activities we offer.

 

Eligibility

To be eligible for support from Middlesbrough Dementia Adviser Service, applicants must: 

  • Be aged 18 or over
  • Require support following a dementia diagnosis

How to apply

To apply directly, please call us or complete the enquiry form below and ask for an application form. We also accept referrals from Middlesbrough Council. All support needs will be assessed before an application is accepted.
